Job Description:
- Responsible for assisting with both hospice and palliative care admissions, to include obtaining intake information and conduct consultations as needed including, provide accurate explanation of the hospice benefit, completion of a physical assessment, and entering the information/documentation into the medical record.
- Responsible for assessing and diagnosing nursing care needs for the Hospice and Palliative Care patients in all age groups; for establishing the nursing outcomes for the Hospice or Palliative Care visit; for prescribing, delegating, and supervising nursing care; for teaching each patient/family and evaluating each patient’s response to nursing care and progress toward agreed upon outcomes.
- Collaboration with the caretaker, physicians, and other healthcare providers involved in the patient’s care to modify the plan of care as needed.
- Provide support to caretaker and other family members in care-taking responsibilities and through the grief process.
- Provide input to the Hospice and Palliative Care Director regarding budget planning, developing departmental policies and procedures and nursing standards of care, as well as other duties as assigned. Must have the ability to function independently.
- The nurse must possess effective problem-solving techniques, use effective communication, and demonstrate decision making skills.
- Must be sensitive to the special needs of terminally ill patients and their families.
- Participate in community outreach events to promote growth and understanding of hospice and palliative care concepts.
