Ohio County Healthcare has consolidated
into one online patient portal.

We are excited to introduce the OCH Patient Portal, providing you with secure and convenient access to your health records and the ability to manage your care. Our new system is live as of January 17th, 2025.

Portal Features:

  • Access your health records securely.
  • View and review test results, lab reports, and physician notes.
  • Manage appointments and request prescription refills.
  • Communicate with your care team directly.
  • Find important health information like vaccination history and medications.

How to Get Started

If you’re a current patient of OCH or one of our affiliated clinics, you can self-enroll in the portal:

  1. Click the Main URL above and select "Create Account."
  2. Enter the following information exactly as it appears in your medical record:
  • Last name
  • First name
  • Date of birth
  • Last four (4) digits of your Social Security number
  • Email address on file with OCH

Once your information is verified, an email will be sent with a link to complete your account setup.

Need Help?

  • If you encounter issues, such as incorrect information in our system, please contact our Health Information Management (HIM) department or your provider’s clinic to update your records.
  • For assistance with the portal, including creating proxy accounts for minors or troubleshooting login issues, please reach out to our support team.

Note: Do not use the portal for urgent medical matters. In case of an emergency, call 911 immediately.
