Ohio County Healthcare has consolidated
into one online patient portal.

We are excited to introduce the OCH Patient Portal, providing you with secure and convenient access to your health records and the ability to manage your care. 

Portal Features:

  • Access your health records securely.
  • View and review test results, lab reports, and physician notes.
  • Manage appointments and request prescription refills.
  • Communicate with your care team directly.
  • Find important health information like vaccination history and medications.

How to Get Started

If you’re a current patient of OCH or one of our affiliated clinics, you can self-enroll in the portal:

  1. You will need to have the following information:
  • Last name
  • First name
  • Date of birth
  • Last four (4) digits of your Social Security number or your Medical Record Number
  • Email address on file with OCH

If you have all of those items, click the "Enroll" link below.

Once your information is verified, an email will be sent with a link to complete your account setup.

Frequently Asked Questions

Why can't I log in?

1. You must use the exact name given at registration. Refer to your Registration information or ID bracelet to confirm.
2. You must use the email address given at registration
3. If you did not provide your email at registration, or haven't give one previously, you need to contact OCH at (270) 298-7411 to have an email address added to your account.

I received an email with my temporary username and password, but still can't log in?

1. Please verify you are entering the information correctly
2. Passwords are case sensitive
3. Username and password MUST be typed in. They cannot be copied and pasted.
4. If you continue to have problems, we can reset your temporary password and generate a new email with the information.

  • If you encounter issues, such as incorrect information in our system, please contact our Health Information Management (HIM) department or your provider’s clinic to update your records.
  • For assistance with the portal, including creating proxy accounts for minors or troubleshooting login issues, please reach out to our HIM team at 270-298-5422
