Case Management and Discharge Planning

Reviews for admission and continued stay criteria
Communicates necessary information to various insurance carriers and payors
Assists the patient in evaluating support services, family support and the patient's ability for compliance with their plan of care
Provides early intervention in cases where patients are unable to return home or to their previous level of care
Identifies need for financial assistance and provides guidance.

Participates in interdisciplinary team meetings with all disciplines involved in your care to facilitate efficient, appropriate and prompt delivery of health care services
Evaluates all swing bed referrals to determine appropriateness of admission
Provides intake of all referrals for the outpatient infusion/injection program and assists to coordinate care of services
Provides emotional support and works with the chaplain program for those with spiritual needs
Coordinates all discharge needs to help facilitate a smooth discharge from the hospital
